As per the University Policy for Education Abroad and Related Activities in Sites with U.S. Department of State Travel Warning/Travel Alert, individuals traveling for University business/sponsored activities to locations designated by the US Department of State with a Level 3 or 4 Travel Advisory must complete a Waiver Application for Travel to High-Risk Countries/Regions. As part of the Waiver Application, travelers will be asked to describe their travel plans and provide a safety and security assessment including emergency contacts in the travel location. After submission, the form will be routed for Department Head and Dean review, followed by review from the Travel Advisory Committee and the Global Affairs Administration.
